- LOW Plan – Perfect for those who primarily need preventive care like cleanings and occasional basic services. This plan provides strong coverage at a lower out-of-pocket cost.
- HIGH Plan – Designed for employees who anticipate more extensive dental needs. This plan offers enhanced benefits, including adult orthodontia coverage, newly added for 2026. While the premium is higher, it delivers greater flexibility and comprehensive care.
Finding an In-Network Dentist
To maximize your benefits, check if your dentist is in-network by using the Guardian provider search tool: Find a Dentist
Even if your dentist is out-of-network, you still have coverage! Just submit a claim form to Guardian for reimbursement. A copy of the claim form is available below.
